What to bring For Registration

Please have these items on hand when you arrive for registration:

• Drivers License
• Insurance card(s)
• Medicare Card and/or Medicaid Card
• List of medications you are currently taking
• Advanced Directive if you have one

What to Bring For Your stay

We ask that you only bring essential toiletries; i.e. pajamas, robe and non-skid slippers. However, large amounts of money, jewelry or valuables should be sent home or deposited in the hospital safe until discharge. Dentures, eyeglasses and contact lenses should be kept in protective containers when not in use. The hospital cannot be responsible for valuables kept at your bedside..
