Our patients, or others on their behalf, have the right to request assistance with discharge planning. Our Case Management Department is staffed seven days a week (generally 8:00 am to 4:30 pm Monday through Friday, and 7:00 am to 7:00 pm Saturday and Sunday) to assist our patients with discharge planning needs. Our experienced staff of 6 Registered Nurses and 1 Medical Staff Social Worker is available to assist with:

Arrangements for ancillary services such as home health care or home medical equipment

Arrangements for long-term care services such as residential care, nursing home care, skilled nursing facility care or Hospice care

Questions regarding insurance coverage for hospital or post-hospital services

Insurance authorization for hospital and post-hospital services
